Applicant Registration Information
Payment is $300 for training, background check, workbook, certificate, and badge. Payment plans are available. By submission of this application, you accept permission for a background check and our using MailChimp as our main form of group emails. Once you submit your registration form, your application will be processed for approval. Upon approval, you will receive an acceptance email along with an invoice for the registration fee to be payable through Paypal. If you are unable to pay online, please make checks/money orders out to: Crisis Chaplaincy of America and mail to the address above at least 10 days prior to class. Thank you and we look forward to meeting you upon acceptance. All information must be completed in full on the form below.
